Insurance Declaration

I declare that:

  1. The information I have given in this application is true and complete.
  2. I have given all the information that I know or can reasonably get that could affect the underwriter accepting this application, even if I am not sure that the underwriter will use it.
  3. I understand that if I have not given all the information that I know or can reasonably get that is relevant to this application the insurance contract will not be legally valid.
  4. I authorise the AEA or the appointed Brokers, OAMPS Insurance Brokers Ltd, to contact and obtain all details including claims experience from previous Insurers as declared in the above declaration.
  5. I am 18 years old or over or, in any case where the intending AEA member is a minor, am that person's duly appointed legal guardian.
